Daily and As Needed Medication for 2026-2027 School Year

School personnel cannot administer any medication, over-the-counter OR prescription, without a completed authorization form on file in the North Hills office. Please select the appropriate authorization form.

Turn in the proper form with the medication in its original container, to Mrs. Jones in the main office. Students may not carry prescription or non-prescription medicine on them.
